1. Secure an Application/Information Sheet from the office. It is to be accomplished by parents or legal guardians only.
2. Submit the following requirements with the Application/Information Sheet properly accomplished and completed.

Preschool / Early Childhood Education

  • Original copy of Birth Certificate (for presentation and verification)
  • Xeroxed copy of Birth Certificate
  • 1 x 1 ID picture (2 copies)

Elementary – Grade 1

  • Original copy of Birth Certificate
  • Xeroxed copy of Birth Certificate
  • Report Card/Certification that the child is currently enrolled in Preschool

Transferees: Grade 2-6

  • Form 138 (Report Card)
  • Certificate of Good Moral Character
  • Birth Certificate

High School: Same as Transferees

  • Pay the Testing Fee.)
  • Applicants must pass the Admission/Entrance Test.
  • Admission is based on the result of the Entrance Test and the availability of slots.


  1. Secure Enrolment Form (Enrollment Form will only be issued upon presentation of Pupil’s/Student’s Report Card. Accounts Record Card (ARC) is attached to the Enrollment Form.
  2. Fill up the Enrolment Form completely.
  3. Present the Enrolment Form for assessment.
  4. Pay the required fees in the office.

Note: Present the Enrollment Form together with the official receipt for recording.

1. Parents are held responsible for their child. In their absence, they should appoint a guardian to assume their responsibilities. Their utmost cooperation is necessary for the proper training of the child.
2. Parents/ Guardians are enjoined to attend the following:

  • Parent’s Meeting – scheduled before the start of the school year.
  • Parent- Teacher dialog – every June 27, Feast Day of the patroness of the school, Our Lady of Perpetual Help.
  • Parent- Teacher conference – as the need arises.
  • Parent-Teacher Council General Assemblies – once every quarter. It is usually scheduled on the Issuance of Report Card.

3. When the pupil/student is unable to attend classes due to sickness and other serious reasons, he/she must present an excuse letter written and duly signed by the parent/guardian explaining the reason for the absence in the Student’s Handbook.

4. The school does not allow any parent/guardian/helper to stay within the school premises during class hours.

5. The school welcomes parents who wish to confer with any administrator(s) or teachers. Any parent who wishes to confer with the class adviser of his/ her child must set an appointment with the teacher concerned through the Student’s Handbook. Parent-Teacher conference may be done only after class hours or during teacher’s vacant period. During the issuance of cards parents are free to discuss with the teacher adviser their child’s performance.

6. Parents/Guardian who would like their child/children be excused or dismissed early from classes should write the request on the Correspondence Page. It should be presented to the adviser and noted by the Principal/Registrar/Prefect of Discipline. The Correspondence should be presented to the guard and the pupil/student should sign in the logbook at the Guard’s Station.

7. Parents/Guardians and other guests transacting business with the school shall secure a visitor’s pass from the guard after proper identification.

8. Tuition, miscellaneous, and other fees must be paid on designated dates and schedules. The following are the mode and schedules of payment.
Cash (annually) – upon enrollment
Semestral – first sem. – upon enrollment
2nd sem. October

Three Payments – 1st Payment – upon enrollment
2nd Payment – July
3rd Payment – October

Four Payments – 1st Payment – upon enrollment

2nd Payment – July
3rd Payment – October
4th Payment – December

June- February – due within the first five (5) days of the month

It should be made at the office during office hours 8:00-12:00 AM, 2:00-4:00 PM. If for some reasons, payment is delayed, arrangement should be made in the office before the examination, so that the student concerned is not unduly disturbed and tensed for failure to take the examination due to unsettled accounts. Other payments made to the teacher adviser should be placed in the sealed envelope and labeled accordingly. In addition, all monthly dues must be settled within the first five (5) days of the month.

9. Discounts on tuition fees are given as follows:
First Honor/Valedictorian – 50%.
2nd Honor/Salutatorian – 20%
First Honorable Mention – 10%

Note: Honor Graduates from schools with Government Permit may also avail of discounts upon presentation of certification from the school.

• Discounts are also given to siblings enrolled in the current school year.

10. To take the monthly/quarterly examination, pupils/students with settled accounts are issued examination permits duly signed by authorities. Students with pending accounts will not be issued said permit. It will remain in the office until the account has been settled. It will then be forwarded to the teacher adviser for confirmation of examinees.

11. Any refund on Tuition fee will be made in accordance with the School Policy on refunds:
a. If the student dropped before the opening of classes, he/she shall be entitled to a refund of 95% of his/her fee paid.
b. If the student dropped within one week after opening of classes, during the regular year, and within 3 days during summer term, he/she shall be entitled to a refund of 90% of his/her tuition fee paid.
c. If the student dropped during the second week of classes of the regular school year, or after the 3rd day but before the 7th day of summer term, he/she shall be entitled to a refund of 80% in his/her tuition fee paid. 3rd week – 70% refund (30% deductions)

12. The choice of transportation is left to the discretion of the parents. All children however, must be fetched on time. Pupils in the afternoon classes can stay not later than 6:15 PM at which time, the gate will be closed.

13. In accordance with Section XIV, Article 78 of the Manual Regulations for Private Schools, 1995 edition, the School Administration reserves the right to drop, suspend, or expel from school at any time any pupil/student who does not comply with the policies and the principle of school authorities particularly concerning the requirements of good behavior and satisfactory academic work.

14. The Report Card is a document which shows the scholastic performance, deportment, and attendance of the pupil/student. It is issued on designated dates to the parents/guardians only. If for some reasons the parent/guardians cannot come on the scheduled date, they can still get such from the office on the following day after dismissal. Pupil/Student whose Report Card is not taken will not be admitted unless accompanied by the parents/guardians to confer with the principal of her representative. Report Card should be returned two days after the issuance with the parent’s/guardian’s signature. If the card is lost, a fifty pesos fine will be paid. The receipt must be presented to the class adviser before another card will be issued.

15. Accidents happen in spite of precautions; the school cannot be held responsible for accidents happening to students. However, medical aid or assistance is given or hospitalization is provided if necessary.

16. Appointments with the family physician and dentist should be on nonschool days, so that your child does not miss his/her class.

17. Since the student’s handbook serves as the means of communication between the school and the home, parents are enjoined to see their children’s handbook regularly.
